Canadian medicare funds are distributed by the federal government - the "single payer" - to the provinces, who administer their own versions of healthcare.

The federal government sets national health care standards but only sends about 25% of the cost of health care to the provinces. The province pays the rest. In my province, health care is the biggest budget item for the provincial government. (No complaints.)
