I am no expert and I don't endorse any company. I have researched this subject and offer the following information. The best explanation of what really happens when you need air evacuation according to Air Ambulance 1.

You incur injury or illness. Your Doctor deems you need care and air evacuation. The Doctor MUST contact your Hospital in U.S. and arrange for admission. The Air Ambulance company is ten called. They assemble a crew and verify you have a destination hospital bed. Air Ambulance flies down, arranges ground transportation to local airport, flies you back, arranges ground transportation to hospital. Total time something like 12 to 18 hours. Cost ZIH to HOU is $12,500.

That's it in a nutshell. These steps may appear to be somewhat cumbersome but is controlled by international law. The air ambulance must follow these procedures, the onus is on them or they lose their license. They cannot simply take a request, launch a plane, pick up the patient, fly them to U.S. and drop them at the airport. They MUST follow legal procedures and take care of the patient bed-to-bed. I see two issues here when thinking about medical evacuation: 1. The insurance company makes the decision on transport over ground to a nearby facility (they make the decision where you are going) and 2. It may take longer with an insurance company rather than going direct to air ambulance company. And of course, the COST.

And if you are REALLY in trouble, you aren't going anywhere, except maybe by ground ambulance to Acapulco, or Mexico City.

Yep, I'm looking for the quickest way to get to something like a Level 1 trauma ER. From there I MAY need air ambulance to U.S.

If you need a level 1 trauma ER, you just might not have the time it would take to get to the US. Hopefully you could be stabilized here and able to take the time needed.

We had Allianz travel insurance when my husband was diagnosed with appendicitis. He was 4 days into it when he was finally and correctly diagnosed. Dr said he had to go to Mexico city for surgery. Doctor talked with our insurance company and convinced them he would not make it if they sent their crew from the states or if they insisted he go back to a US hospital. Our air ambulance came from Guadalajara and it took several hours before it
arrived but we have never seen the bill for that, the insurance company took care of it directly. Doctor said it was $13,000. The hospital in Mexico City was far nicer and more modern than any of our home town hospitals. We are still dealing with our personal medical insurance for Doctor and hospital bills as it pays first before the travel insurance, but other than it taking a long time I think all is going well. A pretty good value considering we paid less than $100 as it was based on the cost of our air tickets and one of us was flying on a "companion fare"

Skymed lets (makes) you choose your home hospital with their air ambulance for expats so yougo right home to your family/doctors.

A while back, someone was raising money to help a friend pay for a medevac flight to the States, claiming the cost was $30,000.00. Quite a difference.

Yes, the $25-35k is a number I have often seen quoted. I don't know why it is that expensive, maybe the distance? I went straight to the horse's mouth (Air Ambulance 1) and this is what they quoted for ZIH to HOU. That is the bed to bed cost for evacuation and does not include any other costs, ie.., medical facilities and providers.

As of right now, this is the way I see it: If I have an emergency then take me to Hospital Especialdes which is open 24/7. Based on their recommendation I might end up to Acapulco or Mexico City or maybe evacuation to HOU. I'm talking about Those events that require getting to care within the "golden hour".

Every situation is different and will call for different solutions.

The information I gave was for the Local Hospital/Doctor getting a bed in the U.S. and you are the final determinant of exactly where you go.

Every "Medical Evacuation" policy I have researched says something like this in the fine print: The Company will confer with the Hospital to determine to the care needed and will make the final determination of the facility to which you will be evacuated.

It is NOT a simple matter of picking up the phone to the company and ordering an air ambulance. Even if self pay the sending Hospital must coordinate with the receiving Hospital then the Air Ambulance gets into the picture.

A friend of mine paid $30,000 for an air ambulance from Las Vegas to Milwaukee last year. Still arguing about reimbursement from Medicare. An insurance "expert" from Canada has analyzed several plans a few years back and concluded that with all the exclusions it was better for him to self insure.

Based on all the great input I have from this posting and other sources, I have decided SkyMed out of Scotsdale, AZ, is the best fit for me. It is one of the few you can buy that is a Year Round Policy renewable without restrictions. It is focused on the aid ambulance service, not medical costs. I have insurance, Tri Care for Life (for military retirees) that reimburses me for 75% of the medical costs.

Once again I thank everyone who contributed to this effort. Each individual has different needs and SkyMed is the best fit for me.
