Over the years, I have heard the word used many times. I have come to believe that it has usage like SOB in English. That is, the meaning is very contextualized.

Said with a sour look on one’s face to the guy in the bar who is hitting on your girlfriend it is an insult. Said with a smile to a friend who just bested you it is a compliment among friends.
