Vet certified to clear screw worm for USA bound dogs

Well, that was the joke. We need to get paperwork so my cousin's dog can board the plane after we visit Zihua in early March. The dog is a rescue and can't be left alone due to severe separation anxiety. Any help will be greatly appreciated - also anyone who enjoys my dumb pun will be as well.
